Yes. Use the “Submit a Rebate” button at the top of the page. All information must match the form you were given at the time of purchase.
a. Your IMEI number must be at least 15 digits and must correspond with your permanent wireless number and rebate you are submitting.
b. To receive your confirmation e-mail and text message the wireless carrier and promo code must match the form you were given at the time of purchase.

Your rebates are processed and issued within 14 weeks of date received by mypromochoice.com. Please note that you must have continuous service for 90 days for your rebate to be valid.
For Member Benefit rebates, please allow 8-10 weeks for processing time.
If you have a pre-ordered device, your rebate will be issued within 14 weeks from the date you purchase and activate your device.
Checks can be re-issued 30 days after the original issuance and upon verification. We will process a stop payment and re-issue a new check(s). Please validate the address we have for you is correct. If your check has been cashed a response will be sent to you with that information.

For any “Denied” status, you will need to contact Wireless Advocates customer service at customerservice@wirelessadvocates.com. We are a third party vendor and can only issue rebates based on their approval.
We are aware that some Costco members have not received the promotional Shop Card associated with a purchase and activation of a mobile phone through Costco.
